Central Sensitization: A Biology, Not a Diagnosis
Central sensitization refers to the state in which the central nervous system amplifies pain signals from the periphery, not because peripheral tissue injury is ongoing, but because the CNS itself has been recalibrated toward hyper-excitability. Neurons in the dorsal horn of the spinal cord and the somatosensory cortex lower their activation thresholds, expand their receptive fields, and generate pain from stimuli that in a normally calibrated system would not register as painful.
This is not a psychological phenomenon. It is a neurobiological one, with measurable correlates in CSF neurotransmitter profiles, brain imaging patterns, and, increasingly, in the genome.
The Genomic Architecture of Pain Amplification
Serotonin transporter variants (SLC6A4 promoter 5-HTTLPR) influence how rapidly serotonin is cleared from the descending modulatory pain pathways. Short allele carriers show reduced serotonin transporter expression, impairing the descending serotonergic inhibition of spinal pain signals. COMT Val158Met directly affects enkephalin degradation in pain pathways, Met carriers show reduced COMT-mediated enkephalin breakdown and paradoxically reduced mu-opioid receptor activity, a combination that impairs endogenous pain suppression.
Catechol-O-methyltransferase variants were first linked to musculoskeletal pain sensitivity in a landmark 2003 study in Science, and subsequent meta-analyses have consistently confirmed that COMT Met allele carriers show lower pain thresholds and higher fibromyalgia prevalence across diverse populations.
The Mitochondrial Dimension
Fibromyalgia patients consistently show evidence of mitochondrial dysfunction: reduced ATP production in skeletal muscle, elevated oxidative stress markers, and impaired mitochondrial membrane potential. SOD2 Val16Ala and other mitochondrial antioxidant variants create the structural conditions for this dysfunction, a cell that cannot neutralize its own respiratory byproducts is a cell in persistent energetic crisis.
Dr. Rivera began designing targeted nutritional interventions for fibromyalgia patients as early as 2006, drawing on oxidative stress biology years before the genomic evidence base for it was established. The GENEFLAMMATION protocol now maps the full genomic architecture of central sensitization, neurotransmitter metabolism, mitochondrial antioxidant capacity, inflammatory regulatory variants, to build a therapeutic roadmap that addresses pain amplification at its biological root.
