COMT Enzyme and Stress Response Genomics | Decoding Inflammation

One SNP, Two Metabolic Speeds
COMT encodes the enzyme that methylates and inactivates catecholamines, dopamine, epinephrine, norepinephrine, in synaptic tissue and the liver. The Val158Met polymorphism, rs4680, produces a well-characterized functional difference: the Met allele reduces enzyme activity by approximately three- to fourfold relative to Val. Met homozygotes clear catecholamines slowly. Val homozygotes clear them rapidly.
Neither genotype is simply better. Val carriers process stress chemistry efficiently but may run with lower prefrontal dopamine tone under baseline conditions, affecting working memory and executive function. Met carriers maintain higher synaptic dopamine but accumulate catecholamines under stress, creating a slower recovery from the neurochemical signature of threat.
The Methylation Connection
COMT is a methylation-dependent enzyme. Its substrate, the methyl group it transfers to catecholamines, comes directly from SAMe, the universal methyl donor produced by the methylation cycle. When MTHFR function is compromised, SAMe availability falls. When COMT activity is already reduced by the Met allele, the combination creates a compounding deficit: a slower enzyme running on reduced fuel.
This interaction is not theoretical. A 2024 study in Neuropsychopharmacology demonstrated that compound heterozygotes for MTHFR C677T and COMT Val158Met show significantly elevated urinary catecholamine metabolites and self-reported anxiety symptoms relative to single-variant carriers. The genome does not isolate its vulnerabilities. It compounds them.
What This Means for Chronic Inflammation
Elevated catecholamines are not merely psychological. They are pro-inflammatory. Norepinephrine and epinephrine activate beta-adrenergic receptors on immune cells, shifting macrophage polarization toward the M1 phenotype and increasing TNF-α and IL-1β production. The Val158Met variant, particularly in homozygous Met carriers under chronic stress, sustains a peripheral inflammatory tone that mirrors the central neurochemical accumulation.
The GENEFLAMMATION protocol maps COMT alongside MTHFR, MTR, MTRR, and MAO-A to construct a complete neurotransmitter methylation profile. The therapeutic implications, targeted methylfolate dosing, SAMe supplementation, and catecholamine-modulating botanicals, follow directly from the genomic map, not from symptom management alone.
